Return-to-Play Decisions: Are They the Team Physician's Responsibility?

Abstract: Although 148 articles we retrieved mention RTP in relation to a specific injury, medical condition, or specific topic, only 13 articles focused specifically on the RTP decision-making process, and 6 of 13 were restricted to Step 1 of the 3-step model (Medical Factors). Return-to-play is a fertile field for research and thought leadership beginning with a focus on the Team Physician's appropriate role in RTP decision making, particularly considering the factors identified in Step 3 (Decision Modification).

“…In this context, the returnto-play (RTP) decision represents a key point. Even if there is little original RTP research, some authors proposed guidelines or suggestions to help clinicians to make an informed decision as to whether an injured athlete may safely return to practice and/or competition (Matheson et al, 2011;Herring et al, 2012). For example, a recent model developed by Creighton could clarify the processes that clinicians have to apply when making their decisions (Creighton et al, 2010).…”

“…In general, most would agree that aspects of shoulder examination that would be of interest in the decision for return to sport include assessment of glenohumeral ROM, rotator cuff strength, scapular position and strength, and functional performance tests. Matheson et al proposed a three-step return to sports model that is applicable to patients after shoulder [31]. First step is to evaluate the patient functionally with assessment of symptoms, range of motion, strength, and health status.…”

“…Clinicians who are only interested in minimising the number of injuries in their teams will probably not last long in the job because there will always be other factors that other stakeholders, such as the players and the coaching staff, will consider more important. It should also be acknowledged that many decisions with the potential to affect players' health are not taken by medical professionals in isolation but will in most cases be shared decisions between players, coaching staff and clinicians [139,140]. These decisions will often come down to weighing up between the risk that a player will suffer an injury and the possible consequences of such an injury on the one hand and the potential reward that could come from subjecting the player to that risk on the other hand.…”
