Return to work after severe traumatic brain injury: a national study with a one-year follow-up of neurocognitive and behavioural outcomes

Abstract: The objectives were to investigate the frequency of return-to-work (RTW) one year after severe traumatic brain injury (sTBI: Glasgow Coma Scale, GCS 3-8) and to identify which demographic and injury-related characteristics and neurocognitive factors are associated with RTW. This study is part of a prospective national study on sTBI conducted in all four Norwegian Trauma Referral Centres, including patients aged >15 years over a period of three years (n = 378). For the purpose of this study, only pre-employed i… Show more

“…Regarding age, the current study found a negative relationship with employment status, with older age increasing the probability of being unemployed. Although age has been reported with higher (younger age) or lower (older age) probability for employment after a TBI [11,13,14,27], the systematic review by Willemse-Van Son et al [56] suggests an inconclusive level of evidence. Again, this pattern of relationship with employment status is not exclusive for individuals after TBI: in general, unemployed older workers have greater difficulties in finding a new job compared to unemployed younger workers [58].…”

“…In this study, some of the factors, such as pre-injury or post-injury social support/employment support and others that have been reported previously as potential predictive factors, were not considered, such as cognitive functioning following injury [14,57], pre-injury health-related quality of life [49], self-awareness and self-monitoring [53], and occupational status [26], among others. The majority (60.2%) of patients in this study had a mild TBI, as assessed by the GCS (13)(14)(15). This is in line with other studies conducted across all TBI severity levels in patients admitted to trauma hospitals [65].…”

“…Not participating in work or education is associated with both economic consequences and psychological distress. Several studies have been published on return to work after trauma; however, the overall public health perspective tended to be restricted owing to small sample sizes, or focus on selected groups of patients with specific injuries or specific mechanisms of injury. Two large population‐based studies from the Victorian State Trauma Registry in Australia reported long‐term follow‐up data regarding return to work, but included only the most severely injured patients.…”

“…Higher numbers indicate better performance. A composite measure of executive functioning was calculated using the process used by Sigurdardottir et al (2018) as the mean of standardized scores from the CWIT and TMT.…”
