Reverse Activated Hyrax Pressure Appliance for Treatment of a Keloid Located at Auricula Helix

Abstract: Keloids are considered aberrations of the wound healing process, and various treatment regimens are described depending on the morphology and size of the lesion. Compression therapy using custom-made pressure clips or splints is widely used for the treatment of keloids. The most common complication of this therapy is ulceration due to excessive soft tissue pressure, resulting in delays and prolonged treatment time. This article describes the fabrication of a custom-made pressure appliance for the treatment of … Show more

“…[ 4 ] placed a silicone sheet below modified oyster shells of cold resin and combined with intralesional steroids for a duration of 6 months. Yüzbasioglu[ 5 ] used acrylic shells activated by a Hyrax screw for 12 h a day for 6 months. However, the real danger of inadequate patient compliance with increasing device complexity and the risk of localised pressure ulceration with effective pressure range being only between 24 and 30 mmHg cannot be underestimated.…”

“…Various approaches have been described for the treatment of auricular keloids including surgical excision, cryosurgery, corticosteroid injections, laser therapy, and application of pressure for an extended time 8–11 . Moreover, after surgical removal of the keloid, it is recommended to maintain pressure on the surgical site for 4‐6 months 12,13 …”

“…Different techniques to fabricate a keloid stent have been described in the literature 7,13–18 . The present report describes a fully digital workflow technique to custom fabricate an ear stent after excision of the lesion.…”

“…A successful, conservative and innocuous form of treatment have been achieved by applying uniform and adjustable compression for prolonged periods before and after surgical removal of a keloid. [5] Devices capable of applying the necessary pressure have been developed in the form of clips, buttons, and earrings. Pressure therapy alone or in combination with surgery has shown beneficial results in the treatment of auricular keloids.…”
