Review article: lactose intolerance in clinical practice – myths and realities

Abstract: Summary Background  Approximately 70% of the world population has hypolactasia, which often remains undiagnosed and has the potential to cause some morbidity. However, not everyone has lactose intolerance, as several nutritional and genetic factors influence tolerance. Aims  To review current clinical practice and identify published literature on the management of lactose intolerance. Methods  PubMed was searched using the terms lactose, lactase and diet to find original research and reviews. Relevant articles… Show more

“…While improving the nutritional profile of the milk meal component, nutrition standards limiting fat content may have had the unintended consequences of decreasing consumption and increasing waste (47,66,67) . With more than half of US children expected to be of Hispanic, Black or Asian decent by 2019 (68) , populations at increased risk for lactose malabsorption and intolerance (69)(70)(71) , offering non-dairy milks may increase acceptability. At the same time, leaders in the field have questioned dairy intake recommendations (72) .…”

“…When ingested, lactose is hydrolysed in the small intestine into two monosaccharides (glucose and galactose) that are absorbed via active transport (7) . The hydrolysis of this molecule is catalysed by the enzyme β-galactosidase or lactase, which is present in the brush border of the intestinal villi.…”
