Review: Emerging Eye-Based Diagnostic Technologies for Traumatic Brain Injury

Abstract: The study of ocular manifestations of neurodegenerative disorders, Oculomics, is a growing field of investigation for early diagnostics, enabling structural and chemical biomarkers to be monitored overtime to predict prognosis. Traumatic brain injury (TBI) triggers a cascade of events harmful to the brain, which can lead to neurodegeneration. TBI, termed the "silent epidemic" is becoming a leading cause of death and disability worldwide. There is currently no effective diagnostic tool for TBI, and yet, earlyin… Show more

“…The most frequent victims of TBI are infants (< 4 years), young adults (15–25 years), and the elderly (65+ years). There are a variety of emerging diagnostic techniques for TBI, one of the most relevant being OCT [ 61 ]. TBI is associated with retinal ganglion cell loss [ 127 ], as well as reduced retinal vasculature [ 128 ].…”

“…These were divided into three main groups of acute, sub-acute, and chronic phases (Figure 1). The TBI cohort selection was based on an extensive literature overview of recent research on TBI biomarkers (animal and human) and temporal courses studied in a range of biofluids including blood, plasma, CSF, urine, and the key biomarkers of neuroinflammation [3,[38][39][40][41][42][43][44][45]. Biomarkers included in the panel were obtained in the purest form without any active additives, to avoid impacting spectral signatures.…”

“…90 Other biological analyses such as oculomics, which measures nonophthalmological anatomical and physiological biomarkers in the eye, have also shown promise as biomarkers of brain health. 97 In schizophrenia, shrinkage of the retinal neural layers occurs in parallel with illness progression, brain volume loss, and cognitive impairment, and represents a means of assessing pathophysiology and treatment efficacy in patients. 98 The integration of large-scale analysis of datasets from highthroughput sequencing and genotyping technologies, broadspectrum omics studies in combination with the wealth of data from neuroimaging, consortia, repositories, and smartphone apps, can be achieved using artificial intelligence (AI) and machinelearning technologies (eg, support vector machines, modern neural-network algorithms, cross-validation procedures), thus affording new insights into complex patterns in brain, behavior, and genes.…”
