Abstract: An increasing number of gunshot injuries in combination with delays in reaching hospital and in receiving treatment accounted for the high mortality rate in this unselected series.

“…6,7 Patients with a lacerating stab wound may be more likely to present with pericardial tamponade, whereas those with a more jagged tearing of the myocardium caused by a high-velocity injury are more likely to present with myocardial hemorrhage, accounting for differences in mortality. 5,8 Pericardial effusion was not commonly reported in these patients, but in the setting of mass casualty triage of victims of polytrauma, the sensitivity of an echocardiogram may be impaired.…”

“…5 The survival rate of those admitted with isolated slow-velocity cardiac injury has been reported to be greater than 91% 6 ; however, the victim with both high-velocity injury and polytrauma, both of which are associated with increased mortality, was considered routine in the Combat Support Hospital. 6,7 Patients with a lacerating stab wound may be more likely to present with pericardial tamponade, whereas those with a more jagged tearing of the myocardium caused by a high-velocity injury are more likely to present with myocardial hemorrhage, accounting for differences in mortality.…”

“…1,2 Among these patients, those who suffer CPA as a result of cardiac tamponade are sometimes successfully resuscitated by pericardial drainage or pericardiostomy in an ER. 1,3,4 Following resuscitation in the ER, most of them undergo a repair of cardiac injury in an OR. For some patients who cannot be transferred to the OR because of massive bleeding from injured cardiac sites, temporary stapling has been reported to be effective for hemostasis in order to transfer them to the OR.…”
