Review of diagnostic uses of shunt fraction quantification with technetium-99m macroaggregated albumin perfusion scan as illustrated by a case of Osler–Weber–Rendu syndrome

Abstract: Bilateral pulmonary arteriovenous malformations (AVMs) are rare and are often associated with the hereditary hemorrhagic telangiectasia (HHT/Osler–Weber–Rendu) syndrome. We present a woman who presented with neurological symptoms due to a cerebral abscess. On further evaluation, bilateral pulmonary AVMs were identified. The patient was diagnosed with HHT, based on positive family history and multiple cerebral AVMs recognized on subsequent catheter angiogram, in addition to the presence of bilateral pulmonary A… Show more

“…A summary of previous reports of pulmonary arteriovenous fistulas is shown in Table I (9-16). Coil embolization was performed in four patients (10,11,14), surgery in four in vivo 37: 2854-2858 (2023) patients (9,13,15,16), and follow-up in one patient (12). Notably, one patient needed an operation for recurrence after coil embolization (14).…”

“…Hoy en día la arteriografía pulmonar queda reservada para la confirmación diagnóstica con eventual embolización en el mismo acto. Otras técnicas de imagen incluyen los estudios de gammagrafía de perfusión, aunque actualmente en desuso, así como la angioRM para la evaluación morfológica y de perfusión, con la ventaja respecto al TC de no emitir radiaciones ionizantes 17 .…”

“…[23,29] Other imaging modalities include perfusion scan, although it has largely been replaced by the state-of-the-art CT and magnetic resonance imaging imaging for both morphological and perfusion assessment. Perfusion scan may still have a role in assessing shunt ratio in the correct clinical context [30] [Figure 6].…”
