Review of the anatomic concepts in relation to the retrorectal space and endopelvic fascia: Waldeyer’s fascia and the rectosacral fascia

Abstract: The rectosacral fascia divides the retrorectal space into inferior and superior portions. This must be differentiated from Waldeyer's description of the fascia lying in the inferior limit of the retrorectal space, formed by the fusion of the rectal visceral and parietal fascias.

“…During dissection of the lower rectum from above, the initial dissection plane near the upper rectum is developed in contact with the fascia recti and leads to the rectal muscularis below, unprotected by the mesorectum. In order not to touch the tumor, it is necessary to resect the anatomic structures in contact with the muscular wall of the lower rectum: posteriorly, the rectosacral fascia must be separated from the presacral fascia at the level of S3-S4, anteriorly from Denonvilliers fascia, and laterally from the levator ani muscular plane [38,39].…”

“…This fascia meets with the prehypogastric nerve fascia at the pelvic plexus. The most dorsal and caudal part of the parietal presacral fascia is referred to as the Waldeyer's fascia, which fuses with the rectal proper fascia and covers the levator ani muscle at the level of the anorectal junction [30].…”

“…A operação pode, então, ser evoluída em dois tempos (abdominal e perineal) que podem ser desenvolvidos simultaneamente, O tempo abdominal da amputação abdominoperineal para a mobilização do reto segue, atualmente, a sua mais recente modificação que é o uso da técnica da excisão total de mesorreto (ETM) 9,38,48 com a posterior abertura pré-sacaral da fácia de Waldeyer 63 .…”
