Review: The vascular system and erectile dysfunction in diabetes — the role of penile Doppler

Abstract: D iabetes is associated with an increased risk of vasculogenic erectile dysfunction due to arterial occlusive disease and impaired vascular smooth muscle relaxation. Doppler ultrasound of the cavernosal artery offers a functional evaluation of penile haemodynamics, in particular the accurate assessment of arterial insufficiency. The current role of penile Doppler testing in the assessment of diabetic patients with erectile dysfunction is reviewed.

“…In contrast, the 69.3% is higher than the 37% reported among Hong Kong diabetic men [ 19 ] and the 63.6% reported among Chinese diabetic men [ 24 ] but agrees with the 20% to 85% incidence rate for diabetic subjects [ 18 , 20 , 21 ] reported in other studies. This wide variation in the incidence of SD could be due in part to the definition used for SD, the period of data retrieval, the population surveyed, the setting in which the patients were studied, the manner in which the participants were questioned, the number and selection of participants, cultural background, socioeconomic level, quality of psychosexual relationships and income.…”

“…Reduced sexual function is a well-documented complication of diabetes. Previous reports have shown that diabetic men are at increased risk for SD at an earlier age [ 7 , 8 , 18 , 19 ], with an incidence ranging from 20% to 85% [ 18 , 20 , 21 ]. Most of the risk factors for SD (such as vascular disease, hypertension, peripheral neuropathy and obesity) overlap with many of the comorbidities linked with diabetes with prevalence and severity being more common in people with diabetes than in the general population [ 22 ].…”

“…Diabetic vasculopathy including micro‐ and macro‐angiopathy, and endothelial dysfunction plays a major role in causing ED in DM by inducing atherosclerotic changes with a decrease in penile blood flow which is a key factor in vascular ED (Esposito et al, ; Giuliano, Leriche, Jaudinot, & Gendre, ). PPDU is the most widely used investigation to diagnose vascular induced ED, but it is an invasive and expensive (Jones & gingell, ).…”

“…8 PPDU is the most commonly used examination for the diagnosis of vascular induced ED, but it is invasive and costly. 19 Previous studies assumed that platelet and leukocyte adhesion has an adverse impact on erection through the release of vasoconstrictor substances and reactive oxygen species, and this could also play a part in the ED progress. 20 PIs are popular indicators that are used to evaluate the activity and function of platelets.…”
