Revision Hip Arthroplasty: A Histologic Review

Abstract: Over 120,000 hip prostheses are implanted annually in North America. The rate of revision arthroplasty for loosened large joint prostheses is burgeoning because primary arthroplasties are now performed on younger and more physically active patients. Increasingly, pathologists are being confronted with specimens from these reoperations. The histologic descriptions of the inflammatory tissue that surrounds these loosened prostheses stress the histiocytic and foreign body giant cell reaction, the appearance of th… Show more

“…Nanoparticles of polyethylene, metal, or cement debris embedded in the synovium are beyond the resolution of MR imaging but incite a histiocytic and foreign body giant-cell reaction resulting in diffuse nodular thickening of the synovium (15). Histologically, the synovium demonstrates papillary fibrocartilaginous metaplasia and sheets of coagulative necrosis (16). In our experience, a reactive synovitis with a frondlike appearance is most often seen in patients with severe polyethylene wear.…”

“…After MR imaging, 73 (68%) of the 108 patients underwent index revision TKA, and 35 (32%) underwent repeat revision TKA (Fig 1). The left knee was cases, an additional axial inversion recovery sequence was performed with the following parameters: 4000/17; inversion time, 150 msec; echo train length, 16 In the index TKA cohort, there were nine cases that were categorized incorrectly by both readers. Of these, seven cases were categorized as a frondlike synovitis when the surgical diagnosis was nonspecific pain, stiffness, or instability, and six of these had evidence of tissue reaction to particulate implant material at histopathologic examination.…”

“…The intense histiocytic and foreign body giant cell reaction is seen in tissue sections. [3][4][5][6] Metallic debris often is not conspicuous and may be easily overlooked. Metallic particles usually are less than 1 m and ingested by histiocytes.…”
