Revision of the 1990 Working Formulation for the Standardization of Nomenclature in the Diagnosis of Heart Rejection

Abstract: In 1990, an international grading system for cardiac allograft biopsies was adopted by the International Society for Heart Transplantation. This system has served the heart transplant community well, facilitating communication between transplant centers, especially with regard to patient management and research. In 2004, under the direction of the International Society for Heart and Lung Transplantation (ISHLT), a multidisciplinary review of the cardiac biopsy grading system was undertaken to address challenge… Show more

“…TCMR was defined according to the International Society for Heart and Lung Transplantation (ISHLT) 2004 classification (16). ABMR was defined according to the last recommendation of the pathology task force of the ISHLT (17) as follows: pAMR0: no ABMR, pAMR1(Hþ): histopathology positive (microcirculation inflammation, which is defined by the presence of intravascular activated mononuclear cells in the capillaries [IAMC]) and immunohistochemistry negative.…”

“…19 Routine EMB were performed every week for 4 weeks after transplantation, twice per month in the second and third months, then once a month for the next 3 months, and every 3 months until the end of the first year. After the first year post-transplant annual protocol biopsies were carried out.…”

“…AMR is associated with a significantly worse survival rate and predisposes patients to coronary vasculopathy (64). The ISHLT grading system (Table 2) was recently revised to reflect the shift in clinical response to lower grades of rejection and provide recommendations for the histological recognition and immunohistological investigation of acute AMR (65,66).…”
