Revisions of Monoblock Metal-on-metal THAs Have High Early Complication Rates

Abstract: Background A relatively high percentage of monoblock metal-on-metal total hip arthroplasties (THAs) undergo early revision. Revision of these THAs poses challenges unique to this implant type. The early complications after these revisions remain unreported as do the clinical and demographic factors associated with these complications. Questions/purposes We describe (1) the frequency of early complications after revision of monoblock metal-onmetal THA; and (2) the clinical and demographic factors associated wit… Show more

“…The isolated acetabular revisions of MoM components became infected 2% of the time within the first 90 days and 9% at 2 years. With the numbers available, we found no differences between MoM and MoP at any of the examined time points but were higher than other studies [10,22,23,29,32,35,38,42]. These Medicare patients might have had more comorbidities and risk factors for infection than studies from single institutions.…”

“…It has been assumed that isolated acetabular revision of a failed MoM hip would have even higher rates of complications. However, there are several single-institution series of MoM revisions, not specifically focused on complications after acetabular revisions, but these are limited by small numbers of patients [32,38,42]. We asked the following questions: (1) Are there differences in the 30-day medical and wound complications after isolated acetabular revision of MoM and MoP components?…”

“…As there are no other published studies reporting on difficulties of head removal except FDA case reports, we cannot compare our results with those of other studies. In general, revisions of MoM implants have a high risk for complications [15,20]. In the current study, we compared revisions of MoM implants (M 2 a-Magnum TM groups) in which the head could be removed with or without problems.…”

“…Risk factors for component revision were younger age at surgery, higher cup angle of inclination, and female sex. Revisions for ARMD can be complex with extensive bone loss and damage to the surrounding soft tissue encountered intraoperatively and prone to complications and rerevisions [38,47,51].…”
