Rheopheresis in treatment of idiopathic sensorineural sudden hearing loss

Abstract: BackroundOnly few therapeutic options exist for patients with refractory sudden idiopathic sensorineural hearing loss (SISHL). Little is known about the efficacy of second-line therapies. Rheopheresis seems to be an effective therapeutic possibility.MethodsBetween 2012 and 2015, 106 patients with SISHL were enrolled in the study, of whom 52 were refractory to initial treatment. As salvage therapy, these patients were offered either 3 sessions of rheopheresis (33 pts) or intratympanic steroid treatment through … Show more

“…Additional studies have noted hearing recovery at 3 months, 144,224 while others have noted hearing stability from months 2 to 4, 231 months 2 to 6, 232 and months 1 to 12. 233 In a noninferiority RCT comparing oral versus IT steroid therapy in 250 patients with unilateral SSNHL, improvement was seen in the majority (193 of 250, 77%) of cases at 2 months posttreatment and was found to be stable at 6 months for both groups. 232 There was a single outlier patient who showed recovery after 6 months.…”

“…It is a condition with a limited number of partially effective therapeutic options. In one of the six available studies, Kostal et al 17 studied the effect of Rh in patients with steroid-resistant Idiopathic SSHL. Out of 106 patients enrolled with SSHL 52 were refractory to initial treatment and were randomized either to 3 sessions of Rh (33 pts) or intratympanic steroid treatment through MicroWick application (19 pts).…”

“…There was a significant improvement for patients in the Rh arm after the first month and this remained unchanged during the first year, while no improvement was seen in the MicroWick arm. 17…”

“…The incidence in adults varies from 2 to 30 per 100,000 although in Germany, an incidence of as high as 160 per 100,000 individuals per year has been reported (18,19). While the pathogenesis of ISSNHL is not clear, microcirculation disorder, autoimmune disease, cochlear membrane injury, and viral infection (20) have been implicated in its onset.…”
