Rheumatic and other musculoskeletal manifestations and autoantibodies in childhood and adolescent leprosy: significance and relevance

Abstract: Leprosy is an important communicable disease. Despite aggressive control and eradication programs and rapidly developing economies, countries such as India and Brazil continue to be large reservoirs for spread of disease.Leprosy amongst children accounts for approximately 10% of all cases in endemic regions. In 2010, the coefficient of detection of leprosy in Brazil was 18.3/100,000 in the general population and 1.3/100,000 in children (< 15 years of age). 1 The proportion of children (< 15 years of age) was 1… Show more

“…[ 15 ] The clinical manifestations of leprosy are diverse, so the diagnosis may be a greater challenge in non-endemic countries because of low awareness. [ 16 ] Rheumatological presentations of leprosy may mimic RA, spondyloarthropathy or vasculitis. [ 1 ] Clinicians need to enquire about the patient's history, conduct detailed physical examination of the whole body, and combine the laboratory examination with a comprehensive assessment to make the correct diagnosis.…”

“…Among the musculoskeletal manifestations of childhood leprosy are inflammatory chronic arthritis, mimicking juvenile idiopathic arthritis (mostly the poliarticular form) or spondyloarthritis, inflammatory swelling of the hands and feet, neuropathic arthritis, septic arthritis, arthralgias/ myalgias, soft tissue rheumatism and multisystem involvement similar to collagenases, including vasculitis and myositis (CHOPRA, 2014). In addition to systemic symptoms, the presence of autoantibodies may also occur (NEDER et al, 2014).…”
