Rheumatic Manifestations in Autoimmune Liver Disease

Abstract: Autoimmune liver diseases coexist with rheumatic disorders in approximately 30% of cases and may also share pathogenic mechanisms. Autoimmune liver diseases result from an immune-mediated injury of different tissues, with autoimmune hepatitis (AIH) targeting hepatocytes, and primary biliary cholangitis (PBC) and primary sclerosing cholangitis targeting cholangiocytes. Sjogren syndrome is diagnosed in 7% of AIH cases and serologic autoimmunity profiles are a common laboratory abnormality, particularly in the ca… Show more

“…This option appears to be especially attractive in view of the findings which suggest strongly that – in contrast to standard immunosuppression – phage therapy is safe and relatively free of side effects. ALD and rheumatic diseases co‐exist in approximately 30% of cases, suggesting that they may share pathogenic mechanisms 63. In this context, our recent data showing the efficacy of phage therapy in a mouse model of rheumatoid arthritis provide additional arguments to believe that relevant clinical trials should be on the horizon.…”

“…Elevation of liver enzymes is more common in AIH and the presence of certain autoantibodies is more specific to AIH (for example, anti-smooth muscle in type 1 AIH or anti-LKM1 and anti-LC1 in type 2 AIH). On the other hand, some clinical manifestations, like Sicca symptoms in SS or Raynaud or inflammatory arthralgia in RA, may help in the differential diagnosis [74].…”

“…Even though ANA positivity (50-70%) is not particularly helpful in the differential diagnosis versus other autoimmune diseases, certain features like AMA positivity (in almost 100% of cases) or cholestatic pattern in liver function tests are important. This distinction is important, since the mainstay of treatment for PBC is ursodesoxycholic acid, quite different from other autoimmune diseases [74].…”

“…Variability in the characteristics of disease as well as severity had been observed by authors both in JOSpA and AOSpA groups. According to some research studies, JOSpA patients may suffer from severe disease and co-morbidities such as psoriasis, inflammatory bowel disease (IBD), uveitis [8][9][10] , fever [11] , and autoimmune liver disease [12][13] . These differences might be more pronounced in refractory SpA patients.…”
