Background: Primary rhinoplasty outcomes may not meet individual expectations. Consequently, reoperation may be advocated to improve results. This study examines the prevalence of individuals considering revision rhinoplasty, while identifying the main cosmetic and functional complaints and factors associated. Methodology: This is a cross-sectional study conducted in Saudi Arabia using a self-reported online questionnaire distributed through social media channels. The sample included 1370 participants who were all Saudi nationals over the age of 16 who had undergone primary rhinoplasty at least 1 year prior. Results: The prevalence of individuals considering revision rhinoplasty was 44.7%. The primary reason for considering it was the desire for further esthetic improvement in an already acceptable result (50.16%). The most common cosmetic complaints subjectively reported were poorly defined nasal tip (32.35%). The most prevalent nasal function symptom was nasal obstruction (56.9%). Significant factors associated with considering revision rhinoplasty included the physician not understanding the patient's complaints, short consultation time, low monthly income, inadequate information about the expected results, not using computer imaging to predict outcomes, lack of rapport with the surgeon, and inadequate information about the risks and complications. Conclusions: A thorough understanding of patient concerns and expectations, as well as thoughtful consideration of risk factors, may help surgeons achieve more successful outcomes and potentially reduce the incidence of revision rhinoplasties.

Background: Nose thread lifting is a relatively recent technique. It offers a chance to approach nose morphology defects without turning to surgery to obtain a temporary improvement. Nevertheless, it lacks standardization and it can therefore offer quite variable results and short durability. The authors’ experience is presented here along with a recommended methodological approach to deliver reliable techniques for predictable results. Thread lifting techniques for nose reshaping by means of insertion of poly-L-lactic/poly-caprolactone threads are presented, based on principles of graft-based techniques, to obtain temporary morphological correction of selected nose deformities. Methods: A total of 553 patients underwent nose reshaping using poly-L-lactic/poly-caprolactone threads. Of the procedures, 471 were primary treatment, and 82 were secondary treatment after a previous rhinoplasty. The mean follow-up period was carried out through patient photographs and lasted 33.4 months (range 2–60 months). Clinical examinations and patient satisfaction surveys were done at 6 months and 1 year from thread lifting. Results: The Freiburg questionnaire survey (subjective Global Aesthetic Improvement Scale) allowed the authors to confirm a 95% satisfaction rate after 6 months, and 62% at 1 year after treatment. A flow chart based on the recorded results is presented to support operators in selecting the proper correction method to adopt, based on the different listed indications. Conclusions: Nose reshaping techniques with poly-L-lactic/poly-caprolactone threads are presented together with patients’ treatment satisfaction. Standardization is based upon the authors’ experience. Contraindications and encountered complications are discussed, so as to provide the readers with a complete “state-of-the art” presentation of these techniques. In the authors’ experience, this is a reliable and safe approach to obtain temporary amelioration of selected nose defects via a nonsurgical and minimally invasive method.

Dome splitting is effective technique in the nasal tip surgery indicated for the reduction of tip projection, increasing tip rotation, domal arch narrowing and correction of the lobule asymmetry. However, there are concerns regarding its negative effects on nasal airfl ow. The present study compared the effect of rhinoplasty with the dome splitting on the effi cacy of external nasal airfl ow. In a prospective before and after clinical trial, 46 eligible patients (39 females, 6 males) were selected and standardized images of their nasal bases were obtained during normal and deep breathes before and 3 months after the surgery. Patients were subjected to rhinoplasty with open technique and dome splitting was done in the nasal lower cartilage between lateral and medial crura. The area of the nasal external airway was measured during normal and deep breathes before and 3 months after the surgery. The percent of nasal airfl ow efficacy was determined qualitatively and quantitatively in the patients' left and right nostrils using McNemar and Wilcoxon Signed Ranks tests. In the left and right nostrils, 67.4% (31) of the patients, the airfl ow effi cacy was decreased and in 32.6% (15) patients, the airfl ow effi cacy increased. Dome splitting signifi cantly decreased the airfl ow effi cacy of the patients (P<0.05). Airfl ow effi cacy decreased (-7.19±23.7) in the left nostrils than right nostrils (-7.28±14.7). Airfl ow effi cacy of the patients signifi cantly decreased (P<0.05).These results suggested rhinoplasty surgery using dome splitting decreased the nasal airfl ow effi cacy after the surgery, hence care must be exercised for alterations in nasal air fl ow resistance when manipulating the nasal framework is done.
