Right anterior minithoracotomy for aortic valve replacement: 10-year experience of a single center

Abstract: Minimally invasive AVR is a safe procedure, with low perioperative morbidity, and low rates of reoperation and death at late follow-up. Excellent outcomes can be achieved with minimally invasive AVR via right anterior minithoracotomy. Sutureless prostheses facilitate minimally invasive AVR and are associated with reduced operative times.

“…Our study showed mean ventilation time of 6hrs, mean ICU stay of 47.8hrs, mean duration of stay in hospital being 7.133days as compared to ventilation time 9.8 (6) hours; intensive care unit stay was 1.5 (1) days; hospital length of stay was 6.6 (6) days observed by Glauber M et al 3 . Wound complications is decreased in patients operated using the minimally invasive technique, a finding that is confirmed by the review of the scientific literature, patients who undergo minimally invasive procedures have a lower incidence of postoperative infections and mediastinitis 7 .…”

“…Previous studies involving minimally invasive AVR reported less bleeding, early recovery time, less pain, and improved cosmesis. 30 Studies also have reported the costeffectiveness of MIS-AVR with propensity matching and MIS RAT approaches. 31,32 MIS-RDAVR procedures with novel fast-track focused recovery protocols also may prove to be cost-effective.…”

“…Nevertheless, cardiac surgeons have been offering minimally invasive valve surgery (MIVS), both aortic and mitral, for several years with favorable results in the general surgical population [3] -these benefits may be also evident in patients with co-morbidities [4].…”
