Right atrial blood cyst with total occlusion of the right coronary artery

Abstract: Cardiac blood cysts are rarely seen in adult patients and in the right atrium. The origin of cardiac blood cysts is not understood, and several hypotheses have been proposed. We present a rare case of right atrial blood cyst with total occlusion of the right coronary artery (RCA). Inflammatory processes may have played an important role in the development of the cyst, because infiltration of inflammatory cells was observed in the cystic wall. Additionally, total obstruction of the proximal RCA indicated that i… Show more

“…2 In adults, cardiac blood cysts have been reported on the mitral valve, [2][3][4] papillary muscles, 5 right atrium, 6 right ventricle, 7 LV, 8 and the aortic, 9 pulmonic, 10 and tricuspid 11 valves. Reported complications include LV outflow obstruction, 5 occlusion of the coronary arteries, 6 valvular stenosis or regurgitation, [9][10][11] and embolic stroke. 12 The embolic risk of rapid pacing and TAVR in a patient with an intracardiac blood cyst is unknown.…”

“…However, the pathogenesis of a Eustachian valve cyst is presumed to be similar to that of a cardiac blood cyst (4). Cardiac blood cysts are benign cardiovascular tumors that are frequently found in the atrioventricular valves of newborn infants (6). There are two suggested mechanisms underlying cardiac blood cysts.…”

“…Blood cysts are congenital echolucent blood-filled nodules located on the endocardium, particularly along the lines of closure of heart valves [13]. Mobile oval blood cyst can be attached to the right atrial wall causing right coronary occlusion or as a round cyst attached to the inter-atrial septum with a thin, demarcated wall and echolucent core [14,15]. …”
