Right-Sided Sigmoid Diverticular Perforation

Abstract: Diverticulosis is a common disorder among geriatric patients, of whom 10% to 25% go on to develop diverticulitis. Known complications of diverticulitis include formation of phlegmon, fistula, bowel obstruction, bleeding, perforation, and colonic abscess. A less common complication is perforation with formation of an extra-abdominal necrotizing abscess. This case is a report of an 83-year-old female who presented to the emergency department with a necrotizing abdominal wall abscess secondary to right-sided dive… Show more

“…Most patients with diverticula of the right side are asymptomatic [ 6 ]. Right-sided diverticulitis presents with right lower quadrant abdominal, nausea, and vomiting as well as other symptoms and thus can mimic appendicitis [ 1 , 7 – 9 ]. Nirula and Greaney presented a series of 12 cases of right-sided diverticulitis over a 10-year period in a single hospital, in which the preoperative diagnosis of appendicitis was made in 11 cases.…”

“…An inflammatory colonic mass of uncertain etiology that is ultimately diagnosed as right-sided diverticulitis may have an initial working diagnosis of a carcinoma [ 8 ]. Complications of right-sided diverticulitis include “phlegmon, intra-abdominal abscess, fistulas involving adjacent organs, and distant septicemia” [ 7 ].…”

“…Therefore, we The CT images revealed variations in the anatomic positions of the descending colon and sigmoid colon as well as sigmoid diverticulitis. Unlike acute appendicitis, medical treatment is often sufficient to treat diverticulitis, although some cases might require surgery (8).…”

“…The differential diagnosis of acute abdominal pain should be made using appropriate imaging modalities, including ultrasound and CT. Ultrasound is a quick, easy, and relatively painless imaging technique, when performed by experienced users, to measure the size and content of abscesses. Ultrasound evaluation is a reliable method for the diagnosis of right-sided diverticulitis, with high sensitivity (91.3%) and specificity (99.5%) (7,8).…”
