Right ventricle remodeling in chronic thromboembolic pulmonary hypertension

Abstract: Chronic thromboembolic pulmonary hypertension (CTEPH) is an underdiagnosed, but potentially curable pulmonary vascular disease. The increased pulmonary vascular resistance in CTEPH is caused by unresolved proximal thrombus and secondary microvasculopathy in the pulmonary vasculature, leading to adaptive and maladaptive remodeling of the right ventricle (RV), eventual right heart failure, and death. Knowledge on the RV remodeling process in CTEPH is limited. The progression to RV failure in CTEPH is a markedly … Show more

“…Our approach yielded simulation results with excellent agreement with measured data and estimated pulmonary arterial stiffness, TAWSS, and OSI distribution in both baseline and CTEPH. Pulmonary arterial stiffness can drive upstream (i.e., right ventricular (Liu et al 2022 )) and downstream (i.e., pulmonary capillary (Wang and Chesler 2011 )) remodeling to accelerate disease progression. Moreover, changes in WSS are relevant to the growth or dissolution of thrombi (Sakariassen et al 2015 ).…”

“…Ultrasound (US) has been demonstrated to be precise, dependable, not involve exposure to ionizing radiation, has high repeatability for measuring muscle quantity in different populations and is available at the bedside. [7][8][9][10] Importantly, ultrasound offers extremely strong reliability and repeatability when performed by ultrasonographers who received formal training [ICC (intraclass correlation coefficient) of 0.97-0.99 for muscle thickness parameter]. 11 Skeletal muscle dysfunction in COPD patients can manifest as abnormal muscle mass of the lower extremities.…”

“…Chronic thromboembolic pulmonary hypertension (CTEPH) is a progressive pulmonary vascular disease caused by single or recurrent pulmonary thromboembolism, leading to elevated pulmonary vascular resistance and maladaptive right ventricular failure ( 1 3 ). Removal of organized thrombotic materials and neointima by pulmonary endarterectomy (PEA) is the gold standard therapy for operable CTEPH ( 4 ), conferring a 3-year survival rate of up to 89% in operated patients ( 5 ).…”
