Hormonal therapy and chemotherapy play a major role in the palliative management of patients with hematogenous metastases from endometrial carcinoma to the liver, lungs, and bones. Radical treatment in patients with metastases to the lung or liver consists of resection of the metastasis combined with chemo- and/or hormonotherapy for metastases to the bones treatment consists of radiotherapy + chemotherapy, for metastasis to the brain treatment consists of resection combined with radiotherapy.

“…A report by Greenwald et al [3] showed 6 of 1,100 gynecological cancer cases to have metastasis to the heart, but none was endometrial in origin. We were able to find several reported cases of cardiac metastasis from endometrial cancer in the past 30 years [4][5][6][7][8], two of which were from endometrial sarcoma [4,8], which are less rare than the cardiac metastasis from endometrial adenocarcinoma, like in our case [9]. As with our case, the cases we found describe metastasis in the right ventricle, two of which also involved right ventricular malignancy, causing obstruction in the right ventricular outflow tract [4,5].…”

We report a case of a 59-year-old woman with right ventricular metastasis of undifferentiated endometrial cancer. Cardiac metastasis from endometrial cancer is a very rare finding. The case demonstrates that undifferentiated endometrial cancer is capable of metastasizing, presumably through a hematogenous route, to unexpected distant organs. These unexpected sites should not be undermined in the restaging and surveillance of these patients.

A 64-year-old woman presented with swelling of her left leg and mild dyspnea [New York Heart Association (NYHA) class II]. Her medical history consisted of a poorly differentiated adenosquamous carcinoma of the uterus (FIGO 1B, G3). To date, the patient was classified disease-free 5 months after curative surgery (hysterectomy and bilateral salpingo-oophorectomy) with postoperative radiotherapy and denied any B symptoms.Ultrasound revealed old iliacal deep venous thrombosis with only mild elevation of D-dimer (1.2 mg/l). Subsequent computed tomography detected diffuse pulmonary embolism as a probable cause of dyspnea and, surprisingly, a large mass in the right ventricle (RV). Echocardiography showed a nearly complete obstruction of the dilated RV with the beginning of obstruction of the inflow tract, moderate tricuspid regurgitation, developing hepatic congestion, and floating parts of the huge intracardiac mass (Figs. 1 and 2).
