Risk Factors forAcinetobacter baumanniiNosocomial Bacteremia in Critically Ill Patients: A Cohort Study

Abstract: Nosocomial bacteremia caused by Acinetobacter baumannii (AB) is of increasing concern in critically ill patients, and the risk factors for this infection are not well established. An inception cohort study in a 40-bed medical and surgical intensive care unit (ICU) at a single institution was conducted during a 2-year period to determine the risk factors for AB nosocomial bacteremia. Risk factors related to the underlying diseases, the clinical picture at admission, and those acquired during the stay in the ICU… Show more

“…The incidence of Acinetobacter blood stream infection in this study was 1.3%, this is a little lower than 2% reported by Michalopoulos and Falagas (2007) and 8.8% reported by Garcia-Garmendia et al (2001b) This difference may be due to the lower number of patients recruited in the study. Prior to the introduction of current advanced resuscitatory devices, Acinetobacter spp were mainly recovered from the urinary tract however with the advent of mechanical ventilation most of the organisms are now being isolated from respiratory tract specimens.…”

“…baumannii, A. calcoaceticus and A. lwoffii are the Acinetobacter species most frequently reported in clinical literature (Garcia-Garmendia et al, 2001b). There is some data to suggest that the proportion of Intensive Care Unit (ICU)-acquired pneumonia cases being found to be due to A. baumannii is on the increase.…”

“…Previous sepsis in ICU and the number of invasive procedures were also related to the development of A. baumannii bacteremia in this unit. 8 A. baumannii can rapidly acquire antimicrobialresistance genes and thus become multidrug resistant.…”

“…Acinetobacter enfeksiyonu için çalışmalarda belirlenen diğer temel risk faktörleri geniş spektrumlu antibiyotik kullanımı, öncesinde enfeksiyon, total parenteral beslenme ve invaziv prosedürlerin varlığı (periferik damar, arteryel ve santral venöz katater, mekanik ventilasyon, nazogastrik tüp, üriner katater) şeklinde belirtilmiştir (16)(17)(18)(19)(20) . Bizim de çalışmamızda saptadığımız risk faktörleri, öncesinde ve sonrasında kültürlerde üreme (Tablo 1), öncesinde antibiyotik kullanmı, idrar sondası, mekanik ventilasyon, nazogastrik sonda, total parenteral beslenme, santral venöz katater, transfüzyon öyküsü gibi invaziv prosedürler şeklinde saptandı.…”
