Risk factors for pancreatitis after pancreatic sphincterotomy: a review of 572 cases

Abstract: This large series of patients undergoing endoscopic pancreatic sphincterotomy provides further evidence that both patient characteristics and technical factors modify the risk profile for post-ERCP pancreatitis. In addition to providing further definition of which patients are at risk, it also suggests that pancreatic duct drainage is an independently significant protective maneuver.

“…Most workers report an incidence of around 12% for post EPS pancreatitis. Placement of a nasopancreatic tube (NPT) or pancreatic stent can reduce this incidence significantly [18] . Restenosis is reported in around 14% of patients on long term follow up [19] .…”

“…Suspected SOD, history of post-ERCP pancreatitis, younger age, female gender, difficult cannulation, multiple contrast injections into the PD, precut and minor papilla sphincterotomy have been identified as high-risk factors [3,9,16,17]. Retrospective and prospective studies have been conducted to determine whether temporary stent placement across the main pancreatic duct orifice lowers the frequency of post-ERCP acute pancreatitis in patients at high risk for this complication [4,[18][19][20][21].…”
