Risk Factors for Urosepsis in Older Adults

Abstract: Objective: To identify factors that predispose older adults to urosepsis and urosepsis-related mortality. Method: A systematic search using PubMed and CINAHL databases. Articles that met inclusion criteria were assessed using the Strengthening the Reporting of OBservational studies in Epidemiology (STROBE) criteria and were scored on a 4-point Likert-type scale. Results: A total of 180 articles were identified, and six met inclusion criteria. The presence of an internal urinary catheter was associated with the… Show more

“…Pseudomonas aeruginosa infection is somewhat problematic; for instance, many immunocompromised patients are subject to P. aeruginosa, which is considered as attenuated bacteria. 11,12 In such patients, P. aeruginosa infection can easily become severe, and P. aeruginosa causing UTI is often refractory and leads to bacteremia or urosepsis. Urosepsis is defined as urological sepsis caused by UTI or urogenital infection.…”

“…More frequent UTI in these cohorts is not merely bothersome; UTI is known to exacerbate MS 12 , lead to confusion and falls in the elderly 13 , and increase the risk of organ rejection in renal transplant patients 14 . Furthermore, catheter-associated UTI carries an increased risk of urosepsis 15 , and bacteriuria in pregnant women is associated with preterm birth and other maternal morbidities 16 .…”

“…Each year in the UK there are 150,000 cases of sepsis in the population, causing 44,000 deaths. 3 UTI is the commonest cause of sepsis in older adults, 4 with urinary sepsis causing an in-hospital mortality of 33% in this age group. 2 In the United States (US), sepsis is the tenth leading cause of death in patients over 65.…”
