Risk Factors in Fatal Cases of Anaphylaxis due to Contrast Media: A Forensic Evaluation

Abstract: Background: Fatalities following contrast medium exposure are extremely rare in clinical routine, though they may occur as an exception. Some may fall under the purview of the inquiring authorities and forensic pathologists due to their in-hospital occurrence. The purpose of this study was to assess the risk factors for anaphylaxis due to contrast medium administration that can be identified in fatal cases. Methods: Fatalities occurred during the course of clinical investigations with contrast media described … Show more

“…In 3/457 (0.6%) compiled mastocytosis patients, an ICMmediated iHR was identified of which one anaphylaxis, although individual studies were small and with incomplete data on premedication use (reviewed in [67•]). Conversely, in patients experiencing fatal anaphylaxis after ICM exposure, no evidence of mastocytosis was found (although only 8/34 cases had a bone marrow evaluation [68]). Hermans et al suggested that in mastocytosis patients receiving ICM, premedication is not necessary unless there is a history of ICM-mediated anaphylaxis or an anticipated high risk for anaphylaxis [67•].…”

“…Older age and multiple previous exposures to RCM increase the risk of having anaphylaxis associated with hypotension. Fatalities have been reported even after the introduction of non-ionic RCM, with most cases lacking predictable risk factors (46). RCM accounted for 27% of fatal drug-induced anaphylaxis (11).…”

“…Contrast agents are an important cause of anaphylaxis. Palmiere and Bonetti reviewed 34 fatalities due to contrast reported in the literature and a further 8 cases in their own hospital network [52]. Post-mortem findings were non-specific but consistent with an IgE-mediated mechanism.…”
