Risk for antipsychotic-induced extrapyramidal symptoms: influence of family history and genetic susceptibility

Abstract: The association of FHpMD and EPS may be linked to the EPS subtype and age of the patient. A common ATP1A3 genomic variation may represent a susceptibility factor for the risk for antipsychotic-induced parkinsonism in an allele-dependent manner.

“…Drug‐induced parkinsonism is also much more common in women, but the reason for this remains unknown, because PD is more common in men. A genetic predisposition has also been suspected, and genomewide analyses have identified a number of polymorphisms in the dopamine and serotonin receptors/transporter in patients with DIP . Interestingly, in rare instances, DIP patients have been found to have single heterozygous mutations of the parkin gene …”

“…A genetic predisposition has also been suspected, and genomewide analyses have identified a number of polymorphisms in the dopamine and serotonin receptors/ transporter in patients with DIP. [25][26][27] Interestingly, in rare instances, DIP patients have been found to have single heterozygous mutations of the parkin gene. 28 Although both DIP and PD are reasonably supposed to be multifactorial disorders in which inherited, yet unproven, predisposition render some patients more susceptible, at the moment no robust evidence supports common risk factors.…”

“…In addition to the epidemiological risk factors, genetic factors may contribute to inter-individual differences in AIP susceptibility (Basile et al 2002;Arranz and de Leon 2007;Greenbaum et al 2007Greenbaum et al , 2009Kasten et al 2011). Research in this field is limited, since many studies have tended to regard different forms of drug-induced movement disorders as a single clinical entity (Al Hadithy et al 2008).…”

“…Similar to this case, most of the cases of extrapyramidal symptoms reported in neonates from Australia 6 and the United States 7 had multiple confounding factors, including concomitant medications such as antidepressants, delivery factors, and social drug use. A genetic predisposition may also have been a confounding factor in the current case, 9 as the mother had experienced an extrapyramidal symptom, which she described as "seizure"-like symptoms, which was treated with benztropine for several weeks before the delivery.…”
