Risk of Acquisition of Genital Herpes Simplex Virus Type 2 in Sex Partners of Persons with Genital Herpes: A Prospective Couple Study

Abstract: In a prospective study of factors associated with genital herpes simplex virus (HSV) type 2 transmission in couples in whom 1 partner had clinical genital HSV and the other did not, 11 (19%) of the 57 history-negative partners had HSV-2 antibody by Western blot at entry. In follow-up (mean, 16 months) of 29 HSV-2-seronegative partners, overall 4 (14%) seroconverted compared with 3 (23%) of 13 HSV-1- and -2-seronegative partners and 1 (6%) of 16 HSV-1-seropositive partners. Since all seroconverters were women, … Show more

“…25 The observed female-male diVerences for HSV-2 prevalence and incidence rates have been documented by other studies. [26][27][28] The diVerence is believed to be due to the greater mucosal surface of the female genital tract, which results in higher risk of transmission to women. Males are also thought to have higher rate of disease recurrence which is likely to make them more infectious 29 : the estimated risk of a susceptible female contracting HSV from an infected male is 80% following a single contact.…”

“…[3][4][5][6] This is important because most sexual transmission of HSV occurs during episodes of subclinical reactivation among persons with unrecognized infection. 4,[7][8][9][10] Virtually all HSV-2 seropositive people have intermittent shedding from the genital mucosa, and most have mild (and hence unrecognized and undiagnosed) disease. 5 In the absence of symptoms, HSV-2 can be detected in the genital tract, by viral culture, on 3% of days for the first year after initial infection, then on 1% of days for the next 2 years.…”

“…8 Several recent developments have changed many of the traditional concepts about this infection. [9][10][11][12][13] Accurate type specific serological tests have been available for the last few years. While the presence of HSV-2 antibodies essentially confirms the previous genital herpes infection, the presence of HSV-1 antibodies indicates previous infection with HSV-1 without identifying the site of infection, which may be oral or genital.…”
