Risk of bleeding in surgical patients treated with topical bovine thrombin sealants: a review of the literature

Abstract: Background: One of the most anticipated, but potentially serious complications during or after surgery are bleeding events. Among the many potential factors associated with bleeding complications in surgery, the use of bovine thrombin has been anecdotally identified as a possible cause of increased bleeding risk. Most of these reports of bleeding events in association with the use of topical bovine thrombin have been limited to case reports lacking clear cause and effect relationship determination. Recent stud… Show more

“…133 In fi ve studies, major bleeding was defi ned as bleeding requiring reexploration [90][91][92]134 ; across these studies, the risk of major bleeding was remarkably consistent, with a median risk of 4.7% (range, 3.1%-5.9%). Thus, we classify most cardiac surgery patients as being at high risk for anticoagulant prophylaxis-related bleeding.…”

“…The 15 Wistar male were anaesthetized with ketamine 15ml/kg, 5cc syringe aspiration in the heart of wistar then collected in 1 glass tube which previously 10% anticoagulant citrate dextrose phosphonate (CPD) buffer (12:15 mg/mL) were given with a ratio of 1 mL of citrate to 5 mL blood in a blood glass tube, put into centrifuge with speed 220 G for 20 minutes. 23 Supernatant layer containing platelet rich plasma (PRP) was taken with a pipette for the second centrifuge with 480 rpm speed for 20 minutes. Pelets from the second centrifuge supernatant was taken and diluted with platelets obtained over 1-1.5 × 1012 platelets/L.…”

“…This range of events was not clearly different from that in the general surgical population, which fell between 0.1% and 20.2%, with most studies reporting rates between 2.6% and 4.0%. 9 Most studies of anticoagulation protein antibodies are conducted in the context of surgery and presume that the presence of antibodies is due to bovine thrombin exposure during the surgical procedure. At present, no community-based population has been tested for anticoagulation protein antibodies.…”
