“…Even though VLN transfer has been shown to be effective in reducing limb volumes, decreasing the number of infectious episodes and improving quality of life (Raju & Chang, ; Ciudad et al, ), donor‐site morbidity has been a source of concern for surgeons. Patients can develop complications such as iatrogenic lymphedema and lymphorrhea among others secondary to flap harvest (Massey & Gupta, ; Pons, Masia, Loschi, Nardulli, & Duch, ; Scaglioni et al, ; Sulo et al, ; Vignes, Blanchard, Yannoutsos, & Arrault, ).…”

“…The omentum contains an abundant supply of lymphatic tissue over a large surface area, making it a viable option for lymphatic reconstruction, especially for patients who have limited peripheral donor site options because of prior surgery, and radiation. It is also an appealing alternative to donor sites such as the groin, axilla, and supraclavicular lymph node basins because it eliminates the risk of donor site lymphedema, which has been reported in peripheral sites . The risk of iatrogenic lymphedema at these donor sites may be minimized with reverse lymphatic mapping, but it requires a technetium injection and involves a significant learning curve.…”

“…However, staying lateral to the artery during flap harvest is not sufficient in terms of safety. Secondary lower extremity lymphatic impairment can still occur after VGLNF . A study describing the anatomy of the VGLNF was completed using magnetic resonance angiography imaging.…”
