Risk of Postoperative Performance Status Worsening after Resection of Lesions Involving the Motor Pathway: A Multinomial Logistic Regression Model

Abstract: In our experience, factors associated with worse functional prognosis and quality of life were a poor preoperative performance status, female gender, operating on a recurrent lesion, involvement of the left corticospinal tract, and surgical or medical postoperative complications.

“…42 In another cohort of 92 patients with gliomas and metastases involving the motor pathway, female sex, among other factors, was associated with a poorer functional prognosis and a poorer quality of life. 43 In a recent cohort of 112 low-grade diffuse glioma, a high fatigue according to the European Organisation for Research and Treatment of Cancer (EORTC) fatigue subscale was noted in 45% of the patients preoperatively and in 42% of the patients post-operatively. Female sex was associated with preoperative fatigue only, whereas male sex was not associated with fatigue.…”

“…The extent of surgery, however, depends on the location of the tumor. For example, patients with gliomas that affect the motor pathway can be susceptible to postoperative paresis or plegia, which would worsen the quality of life ( 9 ). This potential for complications must be taken into account when evaluating the extent of resection.…”

“…Survival after glioma diagnosis is influenced by patient age, IDH mutation status, extent of surgery, location of the tumor, and perioperative Karnofsky performance score (8)(9)(10)(11)(12)(13)(14). Preoperative epileptic seizure is associated with better survival in patients with diffuse glioma (15)(16)(17), whereas cognitive disorder has been associated with worse survival, although not as an independent factor (18).…”
