Risk of significant gynaecological pathology in women with ?glandular neoplasia on cervical cytology

Abstract: At least CIN2 was found in 81.5% in women referred with cervical cytology reporting ?glandular neoplasia. A thorough evaluation of the whole genital tract is needed if colposcopy is negative.

“…Normal colposcopic examination and cervical biopsy may not necessarily be reassuring in glandular abnormalities, as the lesion may be small and may be located higher up in the endocervical canal. Many cases of biopsy-confirmed AIS have no observed colposcopic abnormalities and even combination of cytological testing and colposcopy can miss small endocervical adenocarcinoma or AIS localised in the endocervical canal (Luesley et al , 1987; Ullal et al , 2009; Talaat et al , 2012). Accuracy of colposcopy ranges from 9 to 50% in recognising biopsy-confirmed glandular abnormalities (Ullal et al , 2009; Talaat et al , 2012).…”

Furthermore, the accuracy of colposcopy in predicting high-grade disease, is lower in low-grade cytological abnormality (12%) compared to high-grade cytological abnormality (58% Pretorius et al , 2001).…”

“…Despite marked variation between regions in the incidence of ADC, there is evidence for an absolute increase in ADC . This fact partly reflects the increase in ADC relative to SCC, the inherently poor identification of adenocarcinoma in situ (AIS) by routine cytology and colposcopy, and the variation in the quality and coverage of cytological screening …”

“…Some cancers develop in the background of small foci of cervical intraepithelial neoplasia grade 3 (CIN3) and are inherently more difficult to prevent . Others are adenocarcinomas, which are known to be more difficult to detect as precancerous lesions on cytology and at colposcopy . When women in their 20s and 30s are found to have cancer they are naturally surprised as well as distressed.…”

However, now that regular screening has controlled invasive cervical cancer in older age groups its peak incidence in England is in women between 25 and 35 years of age.…”
