Robotic-Assisted Abdomino-perineal Vesicourethral Anastomotic Reconstruction for 4.5 Centimeter Post-prostatectomy Stricture

Abstract: We report surgical management of a disrupted radical prostatectomy vesicourethral anastomosis after bleeding from undiagnosed hemophilia that required re-exploration, pudendal artery embolization, and urinary diversion with nephrostomy and surgical drains. After referral, the 4.5 cm vesicourethral anastomotic defect was reconstructed with a robotic-assisted abdomino-perineal approach. Intra-abdominal robotic-assisted mobilization of the bladder and perineal mobilization of the urethra permitted a tension-free … Show more

“…In this study, only 1 patient had de-novo stress incontinence (10). To date, there is a single report of robotic combined abdominoperineal BNC repair (11). This patient had a 4.5 cm defect after a complicated open radical prostatectomy.…”

“…Chaitanya et al were able to show an association between higher USS with blood transfusion, complications and higher recurrence risk in patients who underwent visual internal urethrotomy, anastomotic urethroplasty and buccal mucosal graft urethroplasty [25]. Other staging systems such as cystoscopy-based reporting system by Purojhit and colleagues have been used in a case report, however, has not been used in studies or validated externally [26]. Remaining scoring systems including the Gombe urethrographic and ULTRA score have not undergone external validation and hence failed to be adopted more widely by the scientific community.…”

“…Dinerman et al. 29 used surgical robots to assist with bladder urethra anastomosis. Intraperitoneal robotic-assisted mobilization of the bladder and urethra perineum mobilization allow for tension-free bladder urethral anastomosis while avoiding resection.…”

“…Oberviously, with more clearer visualization by using robotic assistance versus conventional standard approach, surgeons feel more comfortable in the control of operative procedures and the post-operative outcomes. Dinerman et al 29 . used surgical robots to assist with bladder urethra anastomosis.…”
