Robotic-assisted versus laparoscopic major liver resection: analysis of outcomes from a single center

Abstract: Background: Debate exists regarding outcomes of robot-assisted versus laparoscopic hepatectomy. We reviewed and analyzed major hepatectomies (resection of 3 Couinaud liver segments) performed in a minimally invasive fashion at a single institution. Methods: From 2011 to 2016, 473 major hepatectomy procedures were performed, of which 173 (37%) were performed in a minimally invasive fashion (57 robot-assisted and 116 laparoscopic). Patient demographics, operating statistics and outcomes were analyzed retrospective… Show more

“…However, some previous studies showed that the blood loss in patients who underwent major hepatectomy via robotic vs laparoscopic approaches was non‐significantly different. Spampinato et al and Fruscione et al reported that the estimated blood loss in robotic and laparoscopic major hepatectomies were 250 and 400 mL ( P > .05), respectively. Similarly, Tsung et al reported that the estimated blood loss in robotic and laparoscopic major hepatectomies was 200 and 300 mL ( P = .527), respectively.…”

“…In previous studies on minor hepatectomy and liver resection for posterior liver segments, robotic surgery showed similar perioperative outcomes compared to laparoscopic surgery . However, studies on a large cohort of patients for robotic hemihepatectomy (RH) vs laparoscopic hemihepatectomy (LH) from a single center are rare …”

“…Robot-assisted surgery has been increasingly employed as an alternative to laparoscopy for MILR, mainly in complex and major liver resections [ 29 , 30 ]. Despite potential advantages, most of the available evidence demonstrated similar results between laparoscopic and robotic liver resections [ 5 , 31 , 32 ].…”
