Robotic extralevator excision of a retrorectal giant aggressive angiomyxoma

Abstract: Aggressive angiomyxoma (AA) is a very rare mesenchymal tumor most commonly found in the pelvic and perineal regions. For the complete excision of retrorectal tumors, with extension through the levator muscle into the ischioanal space, open anterior and posterior approaches are typically required. Herein, we report our experience with robotic excision of a giant presacral AA with extralevator extension into the ischioanal space and extraction via Pfannenstiel incision, which we found to be technically feasible,… Show more

“…Surgical excision is the standard treatment, and it is especially relevant to acquire negative pathologic margins to reduce the local relapse. 2 We report the case of a 47 year-old woman diagnosed with an aggressive angiomyxoma of the pelvis and perineum. A magnetic resonance imaging (MRI) scan revealed a 9 cm infiltrative mass at the level of the lateral wall of the left introitus, extending to the left infravesical space, lateral wall of the vagina and anal sphincter, infiltrating the levator ani muscle and the ischiorectal fossa.…”
