Robotic Right Colon Resection: Evaluation of First 50 Consecutive Cases for Malignant Disease

Abstract: Robotic assistance allows performance of oncologically adequate dissection of the right colon with radical lymphadenectomy and to fashion a handsewn intracorporeal anastomosis as in open surgery, confirming the safety and oncological adequacy of this technique, with acceptable results and short-term outcomes.

“…Previous studies [17-20] have reported mean console times for robotic proctectomies [18, 19], sigmoid colectomies [20] and right colectomies of 180, 100, and 180 minutes, respectively [17, 20]. Although the console time found in the current study was somewhat longer than those reported in previous publications [17-20], only small numbers of patients were included in those previous articles.…”

“…The reason for the longer operative time for the robotic proctectomy group appears to be related to less experience of the surgeon and the surgical team. As Spinoglio et al [9] and D'Annibale et al [17] suggested in their reports, operation times will be reduced with increasing surgeon experience.…”

“…Many factors influence operative times, and these include set-up time, docking time, learning curve, and the type of anastomosis [26]. In terms of right colectomy, either a hybrid technique or repositioning of the robotic cart is required [27].…”

“…They consistently demonstrated the safety and feasibility of RC. 1619,22 So far, 3 retrospective comparative studies 20,22,35 and 1 randomized clinical trial 36 have been published. None of them found significant differences in LOS or complication rate between RC and LC.…”

“…1214 To our knowledge, only limited published data exist on robot-assisted colectomies (RCs). They mainly consist of single-institution early outcome reports 15–19 and retrospective comparative studies on RCs. 2022 These early results have demonstrated that RC, while being equivalent in safety and feasibility, usually incurs a higher cost, even beyond the initial purchase of the robot.…”
