Role of ASCA and the NOD2/CARD15 mutation Gly908Arg in predicting increased surgical costs in Crohnʼs disease patients: A project of the European Collaborative Study Group on Inflammatory Bowel Disease

Abstract: Since CD patients positive for Gly908Arg and ASCA demonstrated higher health care costs, it is possible that measurement of Gly908Arg and ASCA at disease diagnosis can forecast the expensive CD patients.

“…These data suggest a more aggressive course of the disease in carriers of risk alleles. The strongest observed effect for ileal location is consistent with the proposed involvement of ileal Paneth cells in the pathophysiology of NOD2/ CARD15-mediated disease susce ptibility [30,36,37] . NOD2/CARD15 mutations may, thus, abrogate normal Paneth cell behaviour, explaining preferential involvement of the terminal ileum [26] .…”

“…7 Anti-Saccharomyces cerevisiae antibodies have been associated with complicated disease course, increased hospital costs and an increased risk of small bowel surgery in CD. [8][9][10][11][12][13] Saccharomyces cerevisiae is an ascomycetous yeast extensively used in the brewing and baking industry. 14 Therefore, the human alimentary tract is widely exposed to its antigens.…”

“…As a third type of Bbiomarker,^genetic risk factors may be seen because they also have some predictive value for the course of IBD (especially in Crohn's disease) and potentially also for relapse rates [33,[52][53][54][55]. (Genetic) biomarkers for the prediction of the response to therapy are almost only available in the context of azathioprine and 6-mercaptopurine therapy [56][57][58].…”
