(1) Background: Prevalence studies on hospital malnutrition are still scarce in the Middle East region despite recent global recognition of clinical malnutrition as a healthcare priority. The aim of this study is to measure the prevalence of malnutrition in adult hospitalized patients in Lebanon using the newly developed Global Leadership Initiative on Malnutrition tool (GLIM), and explore the association between malnutrition and the length of hospital stay (LOS) as a clinical outcome. (2) Methods: A representative cross-sectional sample of hospitalized patients was selected from a random sample of hospitals in the five districts in Lebanon. Malnutrition was screened and assessed using the Nutrition Risk Screening tool (NRS-2002) and GLIM criteria. Mid-upper arm muscle circumference (MUAC) and handgrip strength were used to measure and assess muscle mass. Length of stay was recorded upon discharge. (3) Results: A total of 343 adult patients were enrolled in this study. The prevalence of malnutrition risk according to NRS-2002 was 31.2%, and the prevalence of malnutrition according to the GLIM criteria was 35.6%. The most frequent malnutrition-associated criteria were weight loss and low food intake. Malnourished patients had a significantly longer LOS compared to patients with adequate nutritional status (11 days versus 4 days). Handgrip strength and MUAC measurements were negatively correlated with the length of hospital stay. (4) Conclusion and recommendations: the study documented the valid and practical use of GLIM for assessing the prevalence and magnitude of malnutrition in hospitalized patients in Lebanon, and highlighted the need for evidence-based interventions to address the underlying causes of malnutrition in Lebanese hospitals.

Evidências crescentes mostraram que os cuidados dietéticos, prestados como parte da abordagem multidisciplinar, foram significativamente clinicamente eficazes e custo-efetivos no manejo da desnutrição e na melhoria dos resultados dos pacientes hospitalizados, como diminuição da mortalidade, aceleração da recuperação e redução do tempo de internação tempo de internação 18,19 .…”

A terapia nutricional (TN) é de extrema importância dentro da unidade de terapia intensiva (UTI), visto que é parte fundamental no cuidado do paciente grave, sendo necessário o conhecimento sobre essa temática pelos profissionais da equipe multiprofissional. Assim, o objetivo desse estudo foi avaliar o grau de conhecimento de médicos, enfermeiros e nutricionistas sobre TN em UTIs. Método: Estudo observacional e descritivo, realizado em um hospital público em São Luís, MA, por meio de análise de questionários com questões sobre temas básicos referentes à TN em pacientes críticos. Os dados foram apresentados por meio de frequências simples ou percentuais. Resultados: Em relação às questões levantadas quanto à TN na UTI, o percentual de acerto foi superior a 70% para a maioria das questões, o que demonstra que há certo conhecimento, sendo importante ressaltar que os questionamentos não incluíram itens sobre nutrição parenteral. A faixa etária dos entrevistados prevalente foi de 30 a 39 anos (60%). Em relação às ferramentas de triagem nutricionais, apenas 35,7% dos médicos e 26,8% dos enfermeiros relataram conhecer, e sobre o tempo considerado aceitável de constipação intestinal na UTI, o percentual de acertos foi de 42,9% entre médicos, 39% entre enfermeiros e 60% dos nutricionistas. Conclusão: Algumas lacunas do conhecimento sobre TN foram identificadas, demonstrando a importância da educação continuada, para ampliação do conhecimento sobre essa temática, a fim de melhorar a atenção nutricional e a qualidade da assistência prestada aos pacientes críticos.

Physicians’ Knowledge of Clinical Nutrition Discipline in Riyadh Saudi Arabia

Background: Nutrition plays a major role in the prevention and management of diet-related disease. With the absence of clinical nutrition dietitians, physicians are considered responsible for prescribing nutritional support. Identifying weaknesses in nutritional knowledge among Saudi physicians may provide guidance to improve their nutritional knowledge. Methods: A cross-sectional study that used an anonymous electronic questionnaire to investigate physicians’ knowledge of the clinical nutrition discipline. In addition to demographics, the questionnaire consisted of 15 questions covering six areas in the clinical nutrition discipline (macro- and micronutrients, nutrition and chronic diseases, nutrition and metabolic diseases, nutrition care process, nutrition support therapy, and research). For continuous variables, independent t-tests and one-way ANOVA were used. Results: A total of 332 had completed the questionnaire and were included in the study. Most of the physicians were Saudi (87%), male (73.5%), aged between 26 and 35 years (63.3%), and without health problems (56.3%). The mean score of the physicians’ knowledge was 5.3 ± 1.97 out of 15. Physicians who reported that they received some sort of nutritional training or course (M = 5.57, SD = 2.08) scored significantly more than physicians who did not (M = 5.10, SD = 1.86); t(330) = −2.174, p = 0.30. Conclusions: Nutrition should be reinforced as an important component of continuing medical education. There is a need for hiring more dietitians in health care settings in Saudi Arabia as an integral part of a multidisciplinary team delivering medical care services.
