Role of endoscopic ultrasound and endoscopic retrograde cholangiopancreatography in isolated pancreatic metastasis from lung cancer

Abstract: A case is reported of a 50-year-old woman with a history of small-cell lung cancer admitted with pancreatic head lesions, discovered during investigation for obstructive jaundice. Endoscopic ultrasound assisted fine needle aspiration of the pancreatic mass was consistent with small cell carcinoma, presenting as an isolated metastasis from the previously diagnosed lung cancer. Endoscopic retrograde cholangiopancreatography (ERCP) showed extrinsic compression and a bile duct stricture, requiring sphincterotomy a… Show more

“…There is no standard treatment approach for MIAP in patients with SCLC. Endoscopic intrapancreatic stent implantation is a palliative solution, but in some cases it can improve performance status of the patients and consequently make them suitable for receiving chemotherapy [ 6 , 20 ]. Radiotherapy can be used for palliation of symptoms.…”

“…Extra-thoracic metastasis of lung cancer is most commonly to the bones, liver, adrenal gland, and intrabdominal lymph nodes [ 9 ]. Pancreatic metastasis is commonly associated with diffuse metastatic disease [ 10 , 11 ] and when metastasis occurs from a lung primary, it is most frequently from small cell lung cancers [ 12 ]. Pancreatic metastasis from lung primaries is a rare occurrence, with one study showing that only 1% of non-small cell lung cancers result in pancreatic metastasis [ 13 ].…”
