Role of Fusion Imaging in Image-Guided Thermal Ablations

Abstract: Thermal ablation (TA) procedures are effective treatments for several kinds of cancers. In the recent years, several medical imaging advancements have improved the use of image-guided TA. Imaging technique plays a pivotal role in improving the ablation success, maximizing pre-procedure planning efficacy, intraprocedural targeting, post-procedure monitoring and assessing the achieved result. Fusion imaging (FI) techniques allow for information integration of different imaging modalities, improving all the ablat… Show more

“…US is one of the most readily available imaging modalities used for guiding percutaneous ablation devices. It is relatively easy to operate, does not impart ionizing radiation, and provides real-time, multiplanar guidance at a low cost [ 10 , 11 ]. However, the use of US is limited by significant operator dependency, difficulty targeting deep structures in patients with obesity, limited visualization of air containing organs (e.g., intestinal loops), and limited utility for the visualization of tumors such as hepatic or iso-echoic renal tumors [ 9 , 10 , 11 , 12 , 13 ].…”

Contrast-enhanced ultrasound (CEUS), in which microbubble contrast agents are used as acoustic enhancers, is an alternative modality that enhances procedure guidance and pre- and post-procedural evaluation [ 14 , 15 ].…”

“…Second, it provides accurate guidance for ablation treatments such as RFA, microwave ablation, and cryotherapy, allowing a precise positioning of the probe into the lesions when poorly evaluable on the conventional US, and is a readily usable tool for treatment effectiveness assessment [ 51 ], as the persistence of post-ablation residual vascularity indicates the need for further ablation and detection of hemorrhagic complications [ 2 ]. CEUS can also be used for lesion follow-up to avoid the administration of any nephrotoxic contrast media and radiation exposure.…”

“…Advancements in computer graphics, 3D image processing technology, and the emergence of fusion imaging (FI) have all improved RFA ( 11 ). FI, which overlaps images from different image sources and combines real-time images of ultrasound with the high resolution of CT/MRI ( 9 ), has been developed.…”
