Role of home blood pressure monitoring in resistant hypertension

Abstract: The definition of resistant hypertension (RHT) has been updated to include failure to achieve target blood pressure (BP) despite treatment with ≥3 antihypertensive drugs, including diuretics, renin-angiotensin system blockers, and calcium channel blockers, prescribed at the maximum or maximally tolerated doses, or as success in achieving the target blood pressure but requiring ≥4 drugs. RHT is a major clinical problem, as it is associated with higher mortality and morbidity than non-RHT. Therefore, it is cruci… Show more

“…226 RAAS involves many aspects of CVD, from ED, inflammation, and oxidative stress in atherosclerosis and hypertension to myocardial remodeling with heart failure development. 227–229 RAAS blockage protects against ED and arrhythmia. 230 In addition to the RAAS, atherosclerosis and VC caused by various uremic metabolites play a role in arterial remodeling and a decrease of elasticity, inducing significantly increased systolic arterial blood pressure.…”

“…Therefore, HBPM may play a complementary role, but cannot replace ABPM in the diagnosis of RH. However, HBPM is useful in improving the long-term management of RH [ 59 , 61 ]. It may provide information on the response to antihypertensive medication and improve drug adherence, ultimately leading to an improved hypertension control rate and decreased medical costs.…”

“…In addition to a higher burden of uncontrolled hypertension, resistant hypertension (uncontrolled BP despite treatment with at least three antihypertensive drugs at maximally tolerated doses or controlled requiring at least four drugs) is more common in NHB adults compared with white adults. Accurate resistant hypertension identification and treatment is essential to address the associated death and disability in NHB adults, and SMBP may assist with hypertension management in resistant hypertension patients [37].…”
