Role of multi-parametric MRI of the prostate for screening and staging: Experience with over 1500 cases

Abstract: ObjectiveContemporary prostate cancer (PCa) screening modalities such as prostate specific antigen (PSA) and digital rectal examination (DRE) are limited in their ability to predict the detection of clinically significant disease. Multi-parametric magnetic resonance imaging (mpMRI) of the prostate has been explored as a staging modality for PCa. Less is known regarding its utility as a primary screening modality. We examined our experience with mpMRI as both a screening and staging instrument.MethodsmpMRI stud… Show more

“…The study and patient elementary characteristics were displayed in Table 1. The target patients were biopsy-naive men in 8 studies [3, 15, 20, 22, 23, 37, 38, 41], and men with at least one previous negative biopsy in 8 studies [21, 25, 31, 34, 39, 42, 44, 45]; in 9 studies [26, 29, 30, 32, 33, 35, 40, 43, 46] the biopsy history of the patients was unclear. In rest studies patients were both biopsy-naive men and men with a history of previous negative biopsy.…”

“…The magnetic field strength was 1.5 T in 17 studies [20, 21, 23, 25, 26, 3032, 3742, 44, 46, 47] and 3 T in 10 studies [3, 15, 22, 24, 27, 3436, 43, 45], respectively. DWI was conducted in 26 studies [3, 15, 2029, 3140, 4244] and DCEI was conducted in 20 studies [3, 15, 2129, 3337, 39, 4244], respectively.…”

“…Through the widespread use of prostate-specific antigen (PSA), earlier detection of prostate cancer (PCa) at lower stages, lower grades, and smaller tumor volumes is feasible. Contemporary PCa screening modalities such as PSA and digital rectal examination (DRE) are two parameters used to predict the detection of PCa ( 1 ). Transrectal ultrasound-guided biopsy of the prostate (TRUS-Bx) is considered the standard of care for diagnosis of PCa.…”

“…Vira and associates [6] examine multiparametric MRI as a predictor of extraprostatic extension and demonstrate some flaws in this new technology. Other topics addressed include the cost of care in metastatic hormone resistant prostate cancer [7] , methods to decrease complications in elderly individuals undergoing urologic surgery [8] , quality of life in patients with chronic pelvic pain [9] , role for robotic surgery in the pediatric urology [10] and the indications of prostate MRI in clinical practice [11] .…”
