Role of muscle damage on loading at the level adjacent to a lumbar spine fusion: a biomechanical analysis

Abstract: The results suggest that the paraspinal muscles of the lumbar spine play an important role in adjacent segment loading of a spinal fusion, independent of the presence of rigid spinal instrumentation.

“…This is why the posterior elements and ligaments are important in the maintenance of spinal stability. It has been established that posterior spinal surgery damages the paraspinal musculature, facet joint, and ligaments, causing limitations of ROM 9,17) . Thus, in clinical practice, postoperative posterior lumbar muscle damage plays an important role in the limitations of ROM.…”

“…(1) Injury stage: muscle injury due to various reasons, local hematoma formation, rupture of myofibrillar membrane, cell mass calcium overflow, increased calcium ion concentration in tissue fluid, activation of calcium-dependent protease and obstruction of mitochondrial respiratory function, and necrotic sites continuous damage. If the tissue is inflamed, macrophages will gather at the injury site and devour the diseased tissue [22]. (2) Repair stage: after muscle damage, the relevant stimulation signals are released, cell growth factors are secreted, and skeletal muscle satellite cells in the resting phase near the injury site are activated.…”

“…эти изменения, так называемая дисфункция параспинальных мышц, могут вызывать острую и хроническую боль в пояснице и дегенеративный спондилолистез [16,17]. Дегенерация смежного сегмента (aSD -ускоренный дегенеративный процесс) является серьезным осложнением, требующим высоких экономических затрат [18]. Внедрение новых технологий позволило развить такое направление, как динамическая стабилизация.…”
