Background: Helicobacter pylori gastritis affects two-thirds of the world's human population. Among various invasive and non-invasive tests, histology play a very important role in detecting H. pylori in gastric biopsies. In histology, for detection of H. pylori, we use different histological staining techniques like routine haematoxylin & eosin (H&E) stain, Giemsa stain, Gimenez stain, and periodic acid Schiff -Alcian blue (PAS-AB) stain.Objective: Aim of our study was to evaluate these different histopathological staining techniques for detecting H. pylori in gastric mucosal biopsies and to determine the sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of H&E stain, Gemenez stain and PAS-AB in the detection of H. pylori in gastric biopsies using Giemsa stain as the reference standard.Materials and Methods: It was a prospective descriptive study design of 45 gastric biopsies of patients having gastritis. This study was conducted at Vivekananda Polyclinic and Institute of Medical Sciences over a period of one year, from March 2021 to February 2022. From each formalin-fixed paraffin-embedded block, four glass slides were prepared and stained with H&E stain, Giemsa stains, Gemenez stain, and PAS-AB stain to detect the presence/absence of H. pylori in gastric biopsies. Sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy were assessed.Results: Various staining techniques for detecting H. pylori in gastric mucosal biopsies were compared. In reference to Giemsa stain results, statistical analysis indicates that the diagnostic accuracy of the Gimenez stain, H&E stain, and PAS-AB stain were 95.6%, 91.1%, and 84.4% respectively. Gimenez stain is confirmed to be better than H&E stain and PAS-AB stain to detect H. pylori in 45 gastric biopsies of patients having gastritis. PAS-AB stain is the worst stain to detect H. pylori in gastric biopsies. Conclusion: Gimenez stain has higher diagnostic accuracy than PAS-AB stain in the detection of H. pylori in gastric biopsy. In fact, Gimenez stain has high sensitivity, specificity and diagnostic accuracy as compared to traditional H&E stain while PAS-AB stain has lower sensitivity, specificity and diagnostic accuracy. Thus, Gimenez stain is also recommended for the detection of H. pylori in gastric biopsy.

Background: Helicobacter pylori colonizes gastric tissue in obese patients and mostly remains undetected clinically, as histological and molecular analysis is seldom ordered in such cases. Objectives: This study aimed to detect the frequency of H. pylori using different techniques in sleeve gastrectomy specimens of obese patients with minimal or no symptoms suggestive of gastritis. Methods: This longitudinal study was carried out at Farooq Hospital Westwood Lahore, Morbid Anatomy and Histopathology Department and Resource Laboratory at the University of Health Sciences, Lahore, Pakistan, from February 2021 to September 2021. This study selected 80 cases who underwent sleeve gastrectomy within six months. Helicobacter pylori was detected by rapid urease test (RUT), modified Giemsa staining, and polymerase chain reaction (PCR) techniques. Results: Most patients (83.7%) were clinically asymptomatic, while 10% had mild and 6.3% had moderate to severe gastritis symptoms. Of the asymptomatic patients, 56.7% of biopsies showed chronic gastritis. Rapid urease test and modified Giemsa staining showed positive evidence for H. pylori in 47.3% of cases, whereas an additional 13.2% of biopsies that were negative on conventional methods showed the amplification of H. pylori DNA by PCR. Patients were discharged with proton-pump inhibitors therapy (40 mg/day) that showed no adverse post-surgical event over a follow-up of six months. Conclusions: Persistent obesity and other socioeconomic factors may lead to colonizing asymptomatic H. pylori infection. More sensitive techniques for detecting H. pylori may be employed in resource-constrained settings for better patient outcomes and to minimize the complications after sleeve gastrectomy.

Background: Currently there are different staining methods available for the demonstration of Helicobacter pylori in gastric biopsies sections. Most of these methods are technically demanding, slow and expensive with varying sensitivity and specificity. Aims: This study was to compare some of these techniques for the detection of H. pylori in routine gastric biopsy tissue sections in order to ascertain their reliability and effectiveness. Methodology: Sixty-five positive and seven negative cases of H. pylori were selected based on the results of rapid urease test, serology test and histopathological examination of the tissue sections. Histopathological examinations of sections stained by Hematoxylin and Eosin staining method (H&E), Giemsa staining method, modified Giemsa staining method, modified McMullen staining method and immunohistochemical staining were performed. Results: The sections were evaluated by two independent observers. The sensitivity and specificity for the techniques were as follow: immunohistochemistry, 100% and 100%; modified Giemsa stain, 96.9% and 85.7%; Giemsa stain, 92.3% and 81.4%; modified McMullen stain, 89.2% and 52.9%; H&E, 90.8% and 77.1%. The interobserver agreement was analyzed by Kappa (k) statistics using SPSS version 23 software. The agreement between the two observers was k: 0.859 (98.4%) for immunohistochemistry; k: 0.777 (96.9%) for modified Giemsa stain; k: 0.533 (95.2%) for Giemsa stain; k: 0.267 (92.1%) for modified McMullen stain; and k: 0.421 (93.7%) for H&E stain. Conclusion: The best results were obtained by the immunohistochemical staining and the Giemsa stainings. Although the immunohistochemistry staining method gives the best result and is very reliable, but it is fairly time consuming and expensive. Therefore, considering the cost, applicability and the reliability of the modified Giemsa or Giemsa stain, we recommend both for the detection of H. pylori in gastric biopsies sections especially in resource constrained laboratories.
