Role of TEFFECTOR/MEMORY Cells, TBX21 Gene Expression and T-Cell Homing Receptor on Type 1 Reaction in Borderline Lepromatous Leprosy Patients

Abstract: In spite of hyporesponsivity to Mycobacterium leprae, borderline lepromatous (BL) patients show clinical and immunological instability, and undergo frequent acute inflammatory episodes such as type 1 reaction (T1R), which may cause nerve damages. This work focused on the participation of T cell subsets from blood and skin at T1R onset. We observed a significantly increased ex vivo frequency of both effector and memory CD4+ and CD8+ T cells in T1R group. Besides, ex vivo frequency of T cell homing receptor, the… Show more

“…The human skin is a homing organ for T cells, both in physiological and pathological conditions, and local signaling by IL-15 is required for the formation of long-lived memory cells [ 48 ]. T1R is characterized by an increase in T cells that suggests intravascular activation at the beginning of reactional episodes [ 49 ]. IL-15 has been involved in the promotion of proliferation and survival of circulating memory T cells, and previous studies have demonstrated that IL-15 strongly induces perforin and granzyme B expression in CD8 + CD103 + CD49a + cells [ 50 ].…”

“…7 The type 1 reaction (T1R), also referred to as reversal reactions, mostly affects borderline patients, as a cause of the increased frequency of antigen-specific CD8 + /CD45RA + T cells expressing the cutaneous leukocyte-associated antigen molecule, both in the blood and inside the lesions. 8 On the other hand, the type 2 reaction (T2R), also known as erythema nodosum leprosum (ENL), is characterized by a sudden exacerbation of the chronic conditions, with the appearance of additional skin lesions, reactivation of former ones, formation of painful nodules, general malaise, and aggravation of neurological damage. An early diagnosis of T2R, together with a suitable therapeutic management, may contribute to reduce the incapacities resulting from these episodes that are often severe.…”

“…The study did not give attention to the memory subsets in leprosy spectrum rather they focused on relapse cases. Recently, a cross-sectional study of memory T-cells among type-1 reactions (T1R) has shown that circulating CD4 + T EM , CD8 + T Ec , and pro-inflammatory cytokines increased at the onset of T1R in BL patients ( 13 ).…”
