Role of the thymus in autoimmune myasthenia gravis

Abstract: Thymic pathologies are frequently associated with myasthenia gravis. Although thymoma is found in both men and women mostly after the age of 40 years, thymic follicular hyperplasia is very common in young women. Thymomas are associated with high autoreactivity and reduced efficiency of tolerance mechanisms, at least partially due to the absence of functional medullary epithelial cells. In follicular hyperplasia, the presence of ectopic germinal centers is evocative of what is described in other autoimmune diso… Show more

“…The role of the thymus in MG pathogenesis is supported by a number of factors, including the high rates of thymic pathology seen in MG patients, correlation between anti-AChR antibody levels and the degree of follicular hyperplasia and favorable outcomes post-thymectomy ( 73 ). Thymic hyperplasia is not uncommon in JMG but thymoma is rare as demonstrated by a number of large cohort studies ( 22 , 74 , 75 ).…”

“…Similarly to thyroiditis, in MG, several antigens are the targets of the autoantibodies, and the disease features depend upon the nature of the antibodies. Patients with anti-AChR, but not with anti-MuSK antibodies, have thymic pathologies, hyperplasia among the young patients, and thymoma among the oldest patients ( 60 ).…”

“…TFH is commonly seen adjacent to thymoma and is likely a major contributor to the elevated AChR antibodies 26 . An intriguing question that arises is whether the MG outcome depends on the presence of TFH associated with thymoma.…”
