Rotational assessment of distal femur and its relevance in total knee arthroplasty: analysis by magnetic resonance imaging

Abstract: ObjectiveTo define the distal femur rotation pattern in a Brazilian population, correlating such pattern with the one suggested by the arthroplasty instruments, and analyzing the variability of each anatomic parameter.Materials and MethodsA series of 101 magnetic resonance imaging studies were evaluated in the period between April and June 2012. The epidemiological data collection was performed with the aid of the institution’s computed imaging system, and the sample included 52 male and 49 female patients. Th… Show more

“…Variations in coronal and sagittal alignment have been widely documented. Variations in axial, or rotational, alignment have been indicated in several previous studies (Viel et al, 2013;Amaranath et al, 2014Loures et al, 2015;Jones et al, 2016). The advantage of this study is that it uses a more accurate technique for measuring the rotational alignment of the trochlear groove.…”

“…Jones et al [ 38 ] used MRI scans and found that “only 24% had an external rotation angle between 2.5° and 3.5° relative to” the posterior condyles. Loures et al [ 24 ] measured a range of 11.75° for the PCL and 14.29° for the APA on MRI scans. Theinpont et al [ 20 ] looked at 2637 3D reconstructions of preoperative CT scans and predicted a “41% risk of malalignment” from referencing the posterior condyles.…”

“…There is some research to suggest that on average, the axes are close to parallel [ 18 , 19 ]. However, the relationship is not consistent, with a wide range of variation [ 20 24 ].…”

“…They found great variability in the shape of the distal femur, and more than 40% of the patients presented dimensions smaller than those of the implants available. However, as described by Loures et al ( 31 ) and Voleti et al ( 32 ) , CT does not consider the thickness of the articular cartilage, which limits the applicability of the method not only in the comparison with the implants but also in the definition of the anatomical standards.…”
