Ruling out lupus anticoagulants with mixing test–specific cutoff assessment and the index of circulating anticoagulant

Abstract: BackgroundLupus anticoagulant (LA) is classified in the antibody family that is recognized in antiphospholipid syndrome. Mixing tests are recommended for LA detection, and either a mixing test–specific cutoff (MTC) or index of circulating anticoagulant (ICA) is used for the interpretation. Although we previously showed MTC had higher sensitivity for LA than ICA, there are few studies investigating specificity.ObjectivesTo investigate specificity of multiple activated partial thromboplastin time (APTT) and dilu… Show more

“…7 There are also other potential options for LA investigation. [22][23][24] Only one mixing test can be used to distinguish LA from HA-inh; however, the inhibitor titer must be measured in the later part of the test. In addition, the inhibitor index was capable of distinguishing LA from HA-inh with high accuracy from the analysis of the shape of clot waveform.…”

“…Mixing studies were performed using 2 types [ 4 , 5 ] : ① Immediate mixing study: patient plasma (C) was mixed (A) with an equal volume of pooled normal plasma (B), and then APTTs were immediately detected. The following formula was used: index of circulating anticoagulant (ICA) = (A–B) × 100/C.…”
