Rumination syndrome: Assessment of vagal tone during and after meals and during diaphragmatic breathing

Abstract: Background: Pathophysiology of rumination syndrome (RS) is not well understood. Treatment with diaphragmatic breathing improves rumination syndrome. The aim of the study was to characterize vagal tone in patients with rumination syndrome during and after meals and during diaphragmatic breathing. Methods: We prospectively recruited 10 healthy volunteers (HV) and 10 patients with RS. Subjects underwent measurement of vagal tone using heart rate variability. Vagal tone was measured during baseline, test meal and … Show more

“…7 Abnormal postprandial vagal tone, gastric accommodation, and gastric emptying do not seem to contribute significantly toward the pathogenesis of rumination syndrome. 8,9 Antral eosinophilia, mast cell infiltration, and increased duodenal intraepithelial lymphocytes have been recently described in pediatric and adult patients with rumination syndrome. 10,11 In clinical practice, patients will present with recurrent regurgitation or vomiting and unless a detailed history is taken with explicit questions, targeted to a positive diagnosis of rumination syndrome, the condition is often misdiagnosed or delayed leading, on occasion, to unnecessary investigations and treatments.…”

“…These physiological modifications might be attributable to voluntary (e.g., a distraction to the abdominal compression and CD contraction) or involuntary (e.g., vagal nerve tone) pathway. However, vagal tone was not found to differ between DB and deep breathing, although only DB reduced rumination episodes [19].…”

“…90 Although some studies reported the efficacy of baclofen and pregabalin on such behavioral disorders, 106,107 cognitive behavioral therapy using diaphragmatic breathing should be attempted first because of the equal or greater efficacy without any adverse effect than the drugs. 91,[108][109][110] We reported that cognitive behavioral therapy reduces excessive SGB sufficiently in almost half of the patients. 91 Our cognitive behavioral therapy program is composed of 5 sessions during 10 weeks.…”
