“…Subsequently, a double-blind multicenter study in CU patients using desloratadine and levocetirizine was published showing improved effectiveness with higher dosing up to 4 times the recommended amount [97]. Although a double-blind placebo-controlled study did not show differences in efficacy between a 10 mg or 20 mg daily dose of rupatadine in CU,[98] a recent study showed that higher doses of this drug are more effective than standard doses [99]. Similar studies have not been performed or verified with other second-generation antihistamines.…”

“…The new generation of H1 blockers also exerts a very important anti-inflammatory action. For example, levocetirizine significantly decreases serum levels of endothelial leukocyte adhesion molecule-1 (ELAM-1) and P-selectin, which may reflect its inhibitory action on neutrophil extravasation into the inflamed skin [36], rupatadine exerts inhibitory action on H1 and PAF receptors, whereas desloratadine, at least in high doses, inhibits mast cell activation [33,34].…”

“…In a double-blind RCT, 334 patients with CSU were randomised to receive rupatadine 10mg, rupatadine 20mg or placebo once daily 21. The mean pruritus score (MPS; on a scale of 0 [none] to 4 [very severe warranting doctor visit]) at baseline was 2.5.…”
