Rupture of a chronic expanding hematoma of the thorax into lung parenchyma

Abstract: emphasizing a possible complication of en bloc resections involving the vertebral bodies, especially after chemotherapy, radiotherapy, or both, might prompt the thoracic surgeon to evaluate a possible solution of continuity of the dura mater during extended lung resections. “…CEH should be managed by complete surgical excision, as incomplete removal may lead to recurrence [12]. In order to achieve complete excision in the current case, we had to perform internal hemipelvectomy and hip transposition as a limb salvage procedure because the lesion had expanded considerably to the acetabulum.…”

“…Almost all instances of CEH previously reported in the English literature have a history ranging from 1 month to 20 years, 1,3 and no lesions have ruptured through the skin. There is a single report of a thorax CEH that ruptured into the lung parenchyma after 24 years, 4 so it is conceivable that other subcutaneous CEHs could break through the skin several decades after their inception. A ruptured CEH complicates the differential, as it is easily confused with a malignant tumor or a vascular lesion.…”
